The 8-Question LASIK Candidacy Screener

This quiz gives you a preliminary idea of whether LASIK may be suitable for you. It is NOT a substitute for a professional eye exam. Only a licensed ophthalmologist can determine your actual candidacy.

Question 1: How old are you?

✓ 21-35: Excellent age range for LASIK — prescription typically stable
✓ 36-50: Still a great candidate, but may need reading glasses later
✓ 51+: LASIK still possible, but discuss presbyopia management with your surgeon
✗ Under 21: Usually too early — prescription may still be changing

Question 2: Has your prescription been stable for at least 12 months?

✓ Yes: Good sign — stable prescription is a key LASIK requirement
✗ No/Not sure: You may need to wait 12 months from your last change before proceeding

Question 3: What is your vision correction need?

✓ Nearsighted (myopia): LASIK works very well
✓ Farsighted (hyperopia): LASIK works, though with slightly lower predictability
✓ Astigmatism: LASIK treats most forms of astigmatism effectively
⚠ Very high prescription: May limit options — discuss alternatives with your doctor

Question 4: Do you have dry eyes?

✓ No/Mild: Good — most patients with mild dryness can still get LASIK
⚠ Moderate: May require pre-treatment — discuss with your surgeon
✗ Severe chronic: LASIK may worsen dry eye symptoms; PRK or alternatives may be better

Question 5: Have you had any prior eye surgeries?

✓ No: No concern — proceed normally
⚠ Yes: Mention this to your surgeon — prior surgery affects candidacy assessment

Question 6: Are you pregnant or nursing?

✓ No: No concern
✗ Yes: LASIK is not recommended during pregnancy or nursing — hormones affect vision and healing

Question 7: Do you have any autoimmune or systemic conditions?

✓ No: Good
⚠ Yes (lupus, rheumatoid arthritis, HIV, diabetes, etc.): Discuss with your surgeon — some conditions affect healing and may disqualify you or require special precautions

Question 8: What are your vision goals?

✓ Reduce or eliminate glasses/contacts: LASIK is designed for this
✓ Better distance vision: LASIK typically achieves excellent distance correction
⚠ Perfect vision guaranteed: LASIK produces 20/20 or better in 96% of cases, but results vary — perfect outcomes cannot be guaranteed

How Did You Score?

✓ Strong Candidate

Mostly checkmarks? You have the key indicators for LASIK candidacy. The next step is a comprehensive in-person screening with a refractive surgeon.

⚠ Possible Candidate

Some caution flags? You may still be a candidate, but will need a detailed screening. There are also excellent alternatives like PRK, SMILE, or EVO ICL.

✗ Consider Alternatives

Several disqualifying factors? LASIK might not be your best option. Ask about PRK, EVO ICL, or other vision correction alternatives that may still free you from glasses.
